Complete the Klavon’s Pizzeria & Pub Donation Request Application below. After the application is submitted, email (if applicable) your organization’s federally issued letter certifying non-profit 501(c)3 status and/or your event flyer. Submit the application at least 3-4 weeks prior to the date the donation is needed.
Or submit an application with the info below in the outline application along with your organization’s federally issued letter certifying non-profit 501(c)3 status and/or your event flyer to the Klavon’s Pizzeria & Pub nearest the event in person, by mail, fax, or email. Sorry, but we are unable to handle any requests on a call-in or walk-in basis without proper paperwork.
Please Note: Due to the enormous amount of donation requests we receive; we cannot respond to phone calls or emails regarding the status of your application. If your donation request is approved, we will contact your organization at least two weeks prior to your event. Each Klavon’s Pizzeria & Pub location has budgetary constraints and can only approve a limited number of the many donation request applications received.

The address of the non-profit generally determines the restaurant reviewing the request. Organizations which are regional should address their donation request application to the restaurant nearest to the location of the event, fundraiser or program.
Correctly completed donation application requests are reviewed in a timely manner. If your application is approved for a donation, you will generally receive notice at least 2 weeks prior to your event, fundraiser or program date.
In order to accept and review your donation request application, it must be accompanied by a copy of the federally issued statement of your organization’s 501(c)3 status (if applicable) and your 9-digit Federal Identification Number. This statement is obtained from the Federal Treasury and is different from a tax-exempt form. For accounting purposes, documentation, including non-profit status, of every donation request application processed must be kept on record for 7 years. If you are not a 501(c)3 group please attach a copy of the event flyer.
Klavon’s Pizzeria & Pub accepts donation request applications from local non-profit organizations with 501(c)3 status or local fundraising events. The event, fundraiser or program must serve the local community. Klavon’s Pizzeria & Pub cannot make donations to political or religious organizations unless the donation will be used for a local, non-secular, community service-related event or program. Klavon’s Pizzeria & Pub does not donate food to individuals, individuals seeking pledges, multiple requests from the same organization or causes outside the Klavon’s Pizzeria & Pub region. Past support does not guarantee future support.
